Yes, he was -- as he ALWAYS did. These are actual video captures from the documentary he was filming near the Great Barrier Reef when he got hit in the chest with a stingray barb.

Paramedics cut his shirt off en route to the hospital, but he was already dead when he got there. The barb had pierced his heart and delivered stingray venom directly into it. It was a one-in-a-million accident.
